If you are in emergency circumstances, you may wish to complete Client Form 2 in order that we may file your case on an expedited basis. Client Form 2. However, this form is not required for the first attorney-client meeting. The attorney will complete with you at the initial client meeting if you have not completed.

Getting Started

Call today to schedule an appointment with an attorney. In order to review your case, we will need the following items:

  1. Pay advices (paycheck stubs) for the last sixty (60) days preceding the filing of your case. For example, if we file the case January, 2011, we will need to file with the court each of your paycheck stubs from November, 2010. If you are self-employed or have been unemployed and do not have any paycheck stubs, an affidavit will be prepared for you stating that you do not have paycheck stubs for that time period.
  2. Proof of income for six (6) months before your case is filed. For example, if your case is filed January, 2011, we will need your income for the following months: July, August, September, October, November and December 2010.
  3. Copy of most recently filed tax return such as 2010 income tax return.
  4. List of all of your creditors with the names, address, and approximate balances. You must list all of your creditor even if you do not want to declare bankruptcy against them. It is a violation of the bankruptcy laws to fail to include all of your creditors. Listing your creditors does not mean that you will lose the property. You have the right to decide how your creditors will be treated in your Chapter 7 case. You may download the form to list your creditors by clicking here: (download pdf creditor list form). In addition, you may order a free copy of your credit report at www.annualcreditreport.com.
  5. You are required to complete a pre-filing counseling course before your bankruptcy case may be filed.

Please contact www.personalfinanceeducation.com and complete your pre-filing credit counseling certificate. Make sure you complete the correct certificate. Only after your case is filed will you be able to complete the pre-discharge certificate.
